Actually, the character of the doctor doesn't dream of killing his wife, but rather her lover (and twice). It's as if he couldn't hurt her, not even in a reverie, no matter how much he was hurt, and would shoot himself before using physical violence against her, which he did. But I did love the film much more after watching it again one day after the first time, which served more as a preparation for the second viewing.
